It's not something that should even be awkward to begin with.
But it is, because of Wendy's hangups. Her face is the second panel just screams shame.

She is acting like they are talking about a fucking cancer diagnosis.

Amy: She's probably embarrassed that she hadn't brought it up before now, when, as I said earlier, she should have had a discussion with him about this from the onset. But she seems to getting on board with Sarah's transition, albeit slowly. She did refer to her child as "Sarah" in her thoughts, which indicates she's come a long way.
